How to Buy a Used CBCT Machine

CBCT systems are expensive new — $80,000 to $300,000 — and the used market is legitimate. But there are real traps. This guide covers what to check before you bid.

Why the used CBCT market is worth taking seriously

A new CBCT system from a major manufacturer costs between $80,000 and $300,000 fully installed. The same system, 5 to 8 years old, in very good condition with documented service history and transferable software, can be purchased for $15,000 to $55,000.

CBCT technology has been mature since roughly 2010. The physics of cone beam imaging have not changed. A 2016 i-CAT FLX produces diagnostically equivalent images to a 2024 system for the vast majority of implant and endodontic applications. The new system has better software, a nicer interface, and a service contract. The used system has the same diagnostic capability at a fraction of the price.

The key is knowing what to check before you buy. Software licensing, installation requirements, and service availability are where buyers get surprised. This guide covers all three.

Software licensing: the most important thing to verify

Before bidding on any CBCT: Contact the manufacturer directly with the unit's serial number and ask whether the software license can be transferred to a new owner, and what that process costs. This is not a small question. Some manufacturers charge $5,000 to $15,000 for license transfer. Some require a new license entirely. A few systems have licenses that transfer at no cost.

CBCT software is what makes the system usable. Without a valid, transferable license, you have an expensive X-ray machine that cannot produce the 3D reconstructions you need for clinical use.

The listing condition report and seller documentation will tell you the software version installed. Your job is to verify, before you bid, what it costs to put that software in your name legally.

i-CAT Next Generation / FLX

One of the most widely sold CBCT platforms in North America. Strong service network, multiple software generations still in active use. i-CAT software (Xoran) has a clear upgrade path. Holds value well.

Watch for: Verify which software generation is installed and whether it is transferable. Older i-CAT Platinum units (pre-2008) are largely obsolete.

Planmeca ProMax 3D / ProMax 3D Max

Premium European system with a strong reputation for image quality. Romexis software is sold per-seat but generally transferable with documentation. Parts and service are widely available.

Watch for: Romexis license transfer must be confirmed with Planmeca directly. Some older ProMax 3D units have sensor aging issues — verify operational status carefully.

Carestream CS 9000 3D / CS 9300

Dentsply Sirona Orthophos SL 3D / XG 3D

Widely installed in the US. Sidexis software has several generations — confirm current version compatibility with your existing Dentsply Sirona workflow.

Vatech PaX-i3D

Pre-purchase checklist

Go through this before placing a bid. These are the items that cause expensive surprises after the fact.

Software license status
Is it transferable to you? Contact the manufacturer before bidding to verify. This is the single most important check.
Service history
Request all maintenance records. A CBCT that has been regularly calibrated and maintained is worth substantially more than one with no records.
Electrical requirements
Verify your operatory meets the power requirements (voltage, amperage, dedicated circuit). Check the installation manual for the specific model.
Room dimensions and ceiling height
CBCT systems have specific clearance requirements. Measure your room before bidding. Some large-FOV systems require more ceiling clearance than standard dental operatories.
Radiation shielding
Your room must be shielded and surveyed by a qualified medical physicist. If you are retrofitting an existing room, get a physicist evaluation before purchasing.
Computer and workstation
CBCT software requires specific computer specs. Older systems may require older operating systems (Windows 7/8) that are no longer supported. Confirm before installing.
State regulatory requirements
Registration or inspection requirements for CBCT installation vary by state dental board. Check your state requirements before taking delivery.
Network and integration
If you plan to integrate with your practice management software or PACS, verify compatibility before purchase. Not all CBCT systems integrate with all software platforms.
